I fully understand the rules, regulations, guidelines and laws governing participation in such a contest as printed in the criteria on page 1 of this letter. I also understand that I am permitted to participate in ONLY ONE approved EXCEPTIONAL SENIOR CONTEST in the approved sport.

I have not played in any non-approved Exceptional Senior Contest in the last year.

Players selected will be posted on www.nyslacrosse.org   web site on Tuesday. May 20 , 2009. Players selected are to report to Canisius College on  June 14 at 5:15 pm for practice. Wear sneakers or turf shoes, school kilt. Shirt will be provided.

This form is to be attached to the roster of the game participants and mailed with the post game report to the Secretary/Treasurer of the section in which the contest takes place.
