EXCEPTIONAL SENIOR CONTEST

I fully understand the rules, regulations, guidelines and laws governing participation in such a contest as printed in the criteria on page 1 of this letter. I also understand that I am permitted to participate in ONLY ONE approved EXCEPTIONAL SENIOR CONTEST in the approved sport.

I have not played in any non-approved Exceptional Senior Contest in the last year.

This form, a check for $10 and a wallet size photo of the applicant is due no later than May 27, 2011.. Please send to:

Players selected will be posted on www.nyslacrosse.org   web site June 1, 2011. Players selected are to report to Williamsville South High School on  June 12 at 3 pm for practice. Wear sneakers or turf shoes, school kilt. Shirt will be provided.

This form is to be attached to the roster of the game participants and mailed with the post game report to the Secretary/Treasurer of the section in which the contest takes place.
